Add camera
Set up logic: sensor as mouse movement, controller as AND, actuator as mouse look.
Enables x and y axis
Set threshold to zero
Play in standalone player
Notice slight threshold{F115191}

I confirm the bug. It is related to resolutions, in window mode, very close or bigger than desktop resolution.
In those cases the horizontal window borders stay out of monitor side limits and then the center of the screen is calculated in a wrong way.

This is still not working properly.

If used on a small windows inside blender it works, but when I full screen the window I have a hard time moving the camera to the sides, sometimes up and down too.
Add camera
Set up logic: sensor as mouse always or mouse movement, controller as AND, actuator as mouse look.
fullscreen the 3d scene, enter game engine and try to move the mouse around.

the fix will be available in blender 2.74 (not in 2.73). Could you download a current daily build (https://builder.blender.org/download/) and check that it works for you?

Thanks Jorge, yes it works with a daily build, I now remember I tested on a daily build from the 6 or 7 of January just before the 2.73 release and it worked also. Never though about it again until I start working with Mouse Look again. I think I just assumed it was fixed since that date and included on the 2.73 oficial release.
