System Information
OS X 10.10.4 GT740

Blender Version
Broken: 2.75 hash:6bdf1d6
Worked: none

Short description of error
I'm working with little meshes, with millimetres... and needed to use bevel but with 6mm width. Thats when I noticed that Bevel is producing really strange results in this specific size level.

Exact steps for others to reproduce the error
Here I just sized down the cube in edit mode to keep the object scale at 1 and marked the corner edges with bevel weight tool in edit mode. Then added a bevel modifier with weight mode and with width value at 6mm(0.006).Bevel_BUG.blend

Think you are simply facing float precision issue - if you want to work in millimeters, rather than scaling down your mesh (even in Edit mode), you should rather scale down your Units Scale setting to 0.001. That way, internally Blender keeps working at its usual scale, while showing you length & co in mm…

@howardt, will let you decide here whether this is bug or numeric imprecision.

I'm pretty sure this is the same issue as in https://developer.blender.org/T45919

Let's wait on the fix for that to confirm that it fixes this.

As suspected, the issue was indeed the same one (line intersection in library said lines were collinear when they weren't), and this has been fixed by Campbells's commit e503e37333 .
