win 10 - 64 bit
NVIDIA GeForce GTX 560 [last driver version]

Looks like REPEAT LAST action doesn't work properly for editing mode. In particular for extrude operation. When you repeat last extion for extrude - it will allways moves towerd first one extrusion oriented movemant and never toward 'normals'. Even if you will choose any others combinations of transform orientation and pivot point Here is I captured video: https://mega.nz/#!sXYlAIzY!OXinB6ZXQ1FSktu0UkLnnOSDKf_5efb6Pt0EEjAM68Q

Open default file. Put a cube. Turn on edit mode. Select any side and extrude it. Then press SHiFT+R[or open Edit>Repeat last/ or Edit>Repeat History and choose last action] - and repeat action - all looks good. Then take any other side of the cube and press SHIFT+R and you will see that extrusion will oriented toward first one extrusion movement. Even if you will choose any other of transform orientation, you will never get correct result. It's works fine in old version of Blender.

Yes, I guess Repeat Last should not repeat the last operator using the resulting orientation, but the re-calculate the orientation again.

it might be a hairy issue to solve though.
