Short description of error
Convert to mesh breaks topology
Exact steps for others to reproduce the error
Open file. convert to mesh issue.blend
Check that topology are clean, it is just subdivided plane:
Select Object -> Convert to -> Mesh
Result:

Simply applying subdivision gives the same effect, but it only happens when viewport subdivisions is set to exactly 2. It doesn't happen with newly created planes.

I was able to reproduce by applying subsurf level 7 to a plane.
Apparently the problem is in the edit mesh overlay.
Interesting that after applying the modifier, if you change the number of edges or faces the mesh returns to normal. But if you return to the previous values. Back to the mess.
