Perhaps the problem is that .to_mesh () returns the mesh without using the mirror modifier. Therefore, this modifier is not taken into account when calculating the circle. Need more research.

This is not an addon problem. This is a Blender problem. In editing mode, when "in Front" is on, Annotations are not drawn on the surface. In this mode Annotation are snapped to the origin, although surface painting is selected.

Such questions often arise.
The behavior of Blender with Apply Scale for a curve is not obvious to people coming from CAD programs.
Therefore, in Blender I would add function for the curves (one button): "Apply Scale and Mean Radius".
It's not a problem. These are the two ways that deserve to be. Sometimes you need the first (built-in). At times the second.
In your case, use the Scale Reset from the built-in Curve Tools addon.

This is not a bug in the addon.
The addon creates the desired grid, but it does not appear in solid mode. Faces are zero size.
To avoid further questions, I added a display of the created object with wire enabled (a custom option).
