Just to inform that on 2.78-testbuild2 (OS X 10.9.5) the viewport smoke preview works fine.

Here is another file showing the same issue:
what's important is that it was created with a version prior to rB5e66827, then, although looking ruffled, it was resaved in rB5e66827 and reopened in rBf097453 where it looks correct again.
This demonstrates to me that there are no errors in the files, but something has changed between rBf097453 and rB5e66827 (perhaps on "Use Modifier Stack"?), and I think it should be addressed before release.

Why not simply bind it to 'F' key or similar, it would be very handy and it will avoid keyboard conflicts.

As stated by hjaarnio, "=" desn't work if keyboard doesn't have the "=" key, nor works the Shift+0.

That's how I figure it (that agrees on what Brecht tells above):
You press G and start to digit the old way, axes,' -', '+' etc, whether you feel the need for entering a computation, you press the proper key ('=') and you continue to type from what you have eventually already typed.

The '=' has very different keys assigned depending on the keyboard layout, so it can be difficult to reach on some configurations, on my keyboard for instance it is Shift+0.

The same issue occurred to me while in dyna-sculpt, undoing an action via the undo stack.
All the sculpting lost till last opening.
