This looks to be caused by using the random integrator.

this could be made to use the same random seed every time, but you will likely still get flickering with camera motion. so not sure if theres some good solution here besides reducing the random value (which increases stepping)

but, with constant integrator i have long render time

The randomized steps size needs many more AA samples to resolve the noise, and Blender Internal only supports up to 16. This file has it set to 8, so it could be increased still, but it's not going to be enough.

I'm not really sure whether it possible to reach convergence without increasing render time comparing with random integrator with few samples.

In any way, wouldn't consider this a bug, just one of TODOs in the BI volume area, mentioned this report from there: http://wiki.blender.org/index.php/Dev:2.5/Source/Development/Todo/Render#Volume_Rendering
