Short description of error
See video below.
To reproduce, set Falloff Angle to 45, and paint (one continuous stroke)
with the brush radius approximately equal to the object.
All by default (2D Falloff and Accumulate is OFF).

Just try to paint in the Vertex Paint mode, with Falloff Angle set to 30, and big brush radius.
Where the stroke overlaps itself takes place artifacts.
Try Sphere or subdivided Cube, not a flat surface.
This seems to happen when there is a surface with a large curvature under the brush.

Can confirm the issue, but not so sure we can do much about that, this looks to me very similar to the terminator problem in rendering… @ideasman42 or @brecht may know better?

The new vertex paint implementation uses a different falloff and projection code (D5975), so it may solve this and other similar brush projection artifacts after it is merged. We may consider this for the implementation of attribute paint.
