Philipp Oeser (lichtwerk) triaged this task as Normal priority.

Without having looked at the code, I assume the vertex normals are taken into account here and without surrounding geometry (edge-only), these can be 'unexpected' sometimes (at least for me -- see T57104 also).
But also with surrounding geometry (I tried extruding the profiles in a manner that doesnt change Z), I am getting the same "non-flat" unexpected profile reported here.

That being said, I am not sure if this can be called a bug (it might be intentional in certain scenarios)?
I would kindly ask @Campbell Barton (campbellbarton) for his opinion, keeping as TODO for the time being...
