Short description of error
Attempts to add a video or audio file into the VSE crashes Blender with EXCEPTION_ACCESS_VIOLATION. Tested with various videos (including Coffee Run) downloaded from YouTube with youtube-dl, and various other video and audio files with different codecs. Crashes with both drag-and-drop and Add > Movie or Add > Sound. Everything works fine in 2.91.2.

Exact steps for others to reproduce the error
Open Blender, open VSE Sequencer, drag video from Explorer to Sequencer.

Cannot reproduce here.

Please try with File → Defaults → Load Factory Settings to see if you still can reproduce this issue.
And please report back if the issue persists even with factory settings.

Load Factory Settings seems to have solved issue. Unable to determine which setting was causing the crash.

I was able to reproduce a crash (on both Linux and Windows) when adding a sound file to the sequencer. It does not happen when using a blank file, but does when my content is loaded. I'm happy to provide a blend file to reproduce the issue if you're interested.AudioSequencer.crash.txt

Was able to reproduce now (in #87010 -- so will merge these reports)
