Short description of error
Sampling colors in Vertex Paint mode is unreliable even in flat shading or when utilizing an emission shader and standard color management (although this reports the correct color some of the time) in both Cycles and Eevee.

Exact steps for others to reproduce the error

  1. With the default cube selected, enter Vertex Paint mode.
  2. Switch the viewport shading to Solid, the lighting to Flat, and the color to Vertex.
  3. Change the brush color to #000003.
  4. Go to the Paint menu and select Set Vertex Colors (or press Shift+K)
  5. Sample the color on the cube by either using the eyedropper from a color picker, or pressing S with the mouse hovering over the cube (doesn't matter if Sample Merged is checked or not)
  6. Check the brush color and see that it has changed to some other color (#010103 on my machine)

I can see the change in color Hex value. Tested on 2.93.0 alpha latest build .

Thanks. If you have a color wheel open somewhere you'll see the dot jump to a different position too.

IIRC, @Jeroen-Bakker has written somewhere that this is due to vertex paint using the old hardcoded color pipeline that has yet to be updated.
