Selection seems to favor the camera lens gizmo when the camera is small in the viewport, as the issue goes away if you turn that gizmo off.

I think that what gives you your 3D view is usually referred to as the viewport camera.
But yeah, it seems as if you can get the Z-axis to glitch at various viewing angles and projection modes.
Perhaps Blender could claim stake to the unused brand name ZPencil ?
- very similar to the existing "GPencil"
- describes it's added functionality (pencil drawing with Z-depth)
- comparable to the familiar ZBrush name 😉
This is probably normal behavior, and is related to the viewport camera's clipping settings.
Lower your 'Clip Start' setting in the View side panel (N panel) and it will probably fix it.
